Doha: The Home Healthcare Service (HHCS) at Hamad Medical Corporation (HMC) has expanded its services to ensure those in need have quick and easy access to high-quality care at home. The service has reached nearly 2,600 patients.
The Home Health Care Service has expanded in several areas including facilities, services and established a Patient’s Call Center, Fatima Al Haddad, Director of Nursing, Home Healthcare Services-Continuing Care Group told The Peninsula.
With the expansion of services and facilities Home Health Care Service accommodates more than 50% patients compared to 2017.
“HMC’s Home Health Care Service has multidisciplinary care teams, including doctors, nurses, Dietician, Respiratory therapist, oral hygienist and patient care assistants, who conduct home visits to perform necessary clinical examinations and review the patient’s care plan,” Fatima Al Haddad, Director of Nursing, Home Healthcare Services-Continuing Care Group told The Peninsula.
“At present we care for nearly 2600 patients living in the community across Qatar, most of whom are aged 60 years and older,” she added.
HHCS, which is based in Hamad Bin Khalifa Medical City, has satellite offices in Al Khor and Al Wakra and Muaither and Duhail helping to serve the public in more areas.
“We have successfully established Early Supported Discharge Service, a nurses led service to support safe transition of care for the acute care patients from in-patient facilities to their home to reduce the bed shortage in the hospital.
A 24/7 Patient’s Call Center provides clinical care support for HHCS patients and their families to timely attend their concerns, thus avoiding unnecessary hospital visits,” said Al Haddad.
“Duhail Satellite was opened and Muaither Satellite has been expanded to serve the increasing number of patients,” she added.
According to Al-Haddad HHCS has successfully achieved Gold Certificate of Planetree Accreditation and it is obtaining JCI Reaccreditation for the fifth time.
All patients cared by HHCS are above the age of (14) years old and does not require continuous medical supervision or 24 hours nursing care; temporarily or permanently housebound that requires skilled nursing and allied health service; has a suitable permanent address, having a reasonable expectation that physical and psychological needs can be met at home; must have a valid residence permit and/or health card; and must have an assigned caregiver and/or responsible family member who will be identified as the care partner.
“Patient with exceptional circumstances shall be considered on a case to case basis upon the approval of HHCS Exemption Committee,” said Al-Haddad.
